Please print the applicable forms and either mail them or fax them to the school you wish your child to attend.

• Parent Engagement Form
• Application for Direction of School Support
• Request for an Interview With the Admission Committee
  (for children who do not hold an automatic right to French-language education)
• Statement of Right to French-Language Education
  (for children who hold the right to be educated in a French-language school)

Please bring your child’s

1. birth certificate
2. baptismal certificate (if available)
3. health card
4. record of immunization

to the school at your earliest convenience.
